- Submit on-line (With our SSL Certificate)
- Download the Application in PDF Format or in electronic form here
- Fill out the application online, print it from your computer then send it to us.
- Be Sure to send us a Copy of your State Resale Certificate.
- Download a state resale form Here. And send it to us.
- After you Submit Your Application track the Status Here
- Please note that direct online ordering is subject to a service charge of $1000 for activation if account is opened and closed without purchasing merchandise to cover the balance on hand.
