- FOR WIN 98:
  When asked what to do with the file check :"Open".
  Follow all the remaining default installation instructions by clicking Next, Next and Finish. If prompted to install Certificate in the root store say OK. Proceed to form at the Step B link below.
- FOR WIN 2000, 2003 or XP:
  When asked what to do with the file check :"Save" as shown.
  Save the file to your desktop folder, then minimize your browser right click on the file choose install certificate from the right click menu.
  Follow all the remaining default installation instructions by clicking Next, Next and Finish. If prompted to install Certificate in the root store say OK. Proceed to form at link below.

The Certificates are visible by going to the Internet explorer: INTERNET OPTIONS>CONTENT>CERTIFICATES>INTERMEDIATE CERTIFICATION AUTHORITIES & INTERNET OPTIONS>CONTENT>CERTIFICATES>TRUSTED ROOT CERTIFICATION AUTHORITIES

You can submit your application on-line now at the link above. Please have all of your business information available. Prepare to copy a tracking number to track your request. Please do not move off of the credit page until you have completed the form.
